How it's used

The humble self-introduction pattern is 名前 + と申します, where 申す is the humble form of 言う. いたします is the humble of する ("do") and doesn't combine with と to give a name. 申し上げます also means "to say" humbly but takes を for content addressed to a superior (e.g. 意見を申し上げる), not と + a name. ございます is the humble/polite form of ある ("there is"), used for things, not for naming oneself.
