〜た上で needs its companion first — 家族とよく相談した上で — so 家族と opens the bracket at the ★ slot.

Practice 〜た上で →動詞た形＋上で means 'after doing X (then do Y)' and marks a prerequisite step; the request 署名してください only makes sense once 確認 is done. うちに means 'while / before ~ ends' and follows ongoing states, not a completed check. 末に means 'finally, after a long process', which overdramatizes a simple confirmation. 際で is not a valid form — the conjunction is 際に.
